1) Print it out onto A4, 100% scale. When you print the other side, make sure to turn the paper upside down (or left side right..)

2) Fold twice. Use stapler or puncher+strings on the corner so that it'll keep in one shape.

3) Cut the top 1mm with a pair of scissors.
